A valid photo ID prior to your visit is required for your medical records.

Please clear an allotted space for the visit and equipment set up. (7x5 foot space is perfect)

All services must be selected based on the appropriate appointment to allow time required for care. 

Please note that parking fees will be added to your visit total.

Securing an appointment spot requires a non-refundable deposit towards your services. 

Prepayment of services is required to reserve appointments. 

Incomplete information when making appointments may delay or cancel the visit. Please make sure to provide any additional information to your scheduled appointment.
